In this role, you will assess speech, language, cognition, and swallowing disorders; develop and implement individualized care plans; and deliver evidence-based therapies in patients’ homes. You will collaborate closely with an interdisciplinary team, educate patients and caregivers, document visits accurately, and support safe, independent living.
